Cochlear implant clinics can obtain additional patient education materials by downloading and printing each piece (preferred) or by sending a request by email:

1.  Download PDF files of the flyer and poster below, as well as vaccination record stickers printed on an Avery label template commonly available at office supply stores.

• Trifold Flyer
• Wall Poster
• Vaccination Record Stickers (print with Avery 5164 labels)

2.  Send an email to CochlearImplantHealth@gmail.com, specifying the individual materials needed and quantity of each.  Options include:

• Trifold Flyers (packs of 50)
• Wall Posters (pack of 2)
• Vaccination Record Stickers (1 sheet includes 6 stickers) 
• If your clinic did not recieve a Complete Patient Education Kit (2 packs flyers, 1 pack posters, 6 sheets of stickers), please specify this in your email request.
